PMS Non-Disclosure Agreement
All Practice Management System (PMS) vendors are encouraged to integrate their systems with HCAI. If you wish to get details about integration, you will need to sign a non-disclosure agreement, indicating that you will not disclose confidential information about HCAI to other parties.

Please scan the completed and signed form, and email it to pmsvendorsupport@hcaiinfo.ca.
If you do not have access to a scanner, you may fax the form to 416-644-3139 and send an email to pmsvendorsupport@hcaiinfo.ca stating that the form has been faxed.
