Re-enrolment Next Steps

Re-enrolling with HCAI is a simple process. To re-enrol as a health care facility in HCAI, however, your health care facility must have been using the system during the previous rollout (2007-2008) and your facility’s effective date must be in Tier 1 or Tier 2.

If you were not using HCAI during the previous rollout, click here.

Use the following steps to re-enrol with HCAI:

  1. Locate your facility name and effective date on the HCAI full Rollout Schedule.
  2. Complete and sign the re-enrolment form.
    1. The Authorizing Officer must sign the re-enrolment form.
  3. Fax the re-enrolment form to HCAI Processing at 416-497-6505 (before the deadline shown in the table below).

HCAI will confirm receipt of your re-enrolment form. Your business will be activated (Approved) on HCAI as a health care facility approximately 2 weeks BEFORE your effective date.

You will begin using HCAI to transmit all Ontario Claim Forms-plans (18s/22s/23s) and invoices (21s)-on your effective date.
